2024 Vital Medication Program – These drugs will be available to members at a $0 cost share without the members having to satisfy their deductibles. Please note this list may not be all-inclusive, is subject to change throughout the year, and some of the drugs may have quantity limits and other clinical requirements.

                             

Dental Plan-The dental plan provides comprehensive coverage for routine care as well as coverage for basic and major dental services.

Vision Plans-The vision plans provide coverage for eye exams and materials (frames, lenses and contacts).

Life & Accidental Death & Dismemberment Plans (Life/AD&D)–Life/AD&D provides a benefit in the event of an eligible employee’s covered death. It provides an additional amount in the event of a covered death or dismemberment as a result of an accident.

Short-term Disability Plans–provides income during a short term disability (STD). STD becomes effective the 1st day for an accident and the 8th day for sickness. The maximum benefit period is 90 days.

Long-term Disability Plans – provides income during a long term disability (LTD). LTD becomes effective later than short term, and payments run for a longer period of time. LTD becomes effective at 180 days.
